Just as we can't remove the fat without repositioning the skin, sometimes we can't reposition the skin without removing the fat.  If we tried to pull the skin up and back out of the front of her neck without removing the fat first, the skin wouldn't move well, because it would just be pushing the excess fat back against the front of the neck.  Fat removal and face lift are a good combination.
