It's time to get out your ruler and measure some angles.  In the before picture, an imaginary line drawn from the front of his chin through the front of his lips to the middle of the base of his nose points far forward, away from his face.  That same line, from chin through lips to nose, is much more vertical in the after picture.  With a projecting nose and a weak chin, the face seems to "point" toward the nose, making the nose appear even more prominent.
