We narrow that arch by placing sutures across the cartilage.  The technique is diagrammatically represented above.  The black line represents the tip cartilage, seen edge on from the base view, with the dome at the top.  The red line is a suture that we placed below the dome, connecting the columellar portion of the cartilage with the lateral portion.  When we tie that suture, shortening the length of the red line, the columellar and lateral portions of the tip cartilage are pulled closer to each other, and the arch at the dome is tightened.

The photos above show the first pass of the suture to accomplish this narrowing.  The forceps (green) at the top of the picture are holding the tip cartilage in a tightened, narrowed position for the needle (white) and the thread (light blue).  The bottom edge of the tip cartilage is highlighted in red, showing you the tightened arch of that cartilage.
