Now, finally, it's time to make the trans-columellar incision.  I make the first half of the incision, starting at the apex of the upside down "V."  My left hand is pulling up on the tip of the nose, and the baby finger of my right hand is pulling down on the columella, to place the columellar skin under tension and make it easier to cut accurately.

The other side of the incision is made.  The fact that the incision can be pulled open a little (above right) is my indication that I succeeded in cutting all the way through the skin.

We mentioned that the columellar skin is very thin.  The surgeon must be careful while making this incision so he avoids nicking the cartilages that reside in the columella, underneath that thin skin.
