Try to follow the edge of her jaw from the chin up toward the ear in her before and after photos.  In the before photo, we lose sight of her jaw line when we hit the jowl.  In the after picture, we can follow the edge of her jaw farther up toward her ear.  That change is one of the main goals of this operation.

Questioner: Drooping tip, cave man like nose after rhinoplasty. Can this be fixed?
Droopy tip. rhinoplasty almost a year ago. Before surgery my smile was my best feature , now worst. I don't look normal when I smile as you can see in pictures it does not look right.When I smile the tip is pointy and cartilidge shows in certain lighting and it droops down like there's no support in the tip.My bridge also looks wider. My dr said he would make it thinner but hasn't .I feel so ugly , can this be fixed ? I'm a petite girl and should have a petite nose not a cave man looking nose.

Dr. Denenberg's answer: Your nose needs to be shortened substantially
Despite what you might hear, shortening a nose -- raising the tip up -- often involves advanced work on the tip cartilages, and every plastic surgeon is not able to make those changes. Things get harder during a revision, but in expert hands, elevating the tip of the nose is one of the most predictable changes to make. See the attached video and Web reference link for examples of what can be done.

You must be certain to see before and after revision rhinoplasty photos of any doctor you are considering for your revision. You absolutely don't want to be in the position of having had two operations, and still a too-long nose.
